Registering with a GP practice

We are happy to register new patients who live within our practice boundary area of North East Edinburgh (area outlined in the map above). Please enter your postcode to check that your address is within the practice boundary before completing the registration forms.

When you register with us, it’s recommended that you provide information to confirm your identity and address. This means you can be matched to your existing NHS records. 

To register permanently, you need to complete a registration form GPR and a patient questionnaire.

  1. Fill out and submit the online combined Application to Register with a General Medical Practitioner - GPR (Scotland) & New Patient Questionnare 

    & upload ID details via eConsult upload image (select "other requests for documents or paperwork")

    or alternatively

  2. Download both a Registration form and Patient Questionnaire complete them and then hand in to the GP practice.
    Please bring along with you two items of identification, one which must include details of your current address.

 

Registering as a Temporary Resident

If you are not staying in the area for more than 3 months you can be seen as a Temporary Resident.

For more information on being treated as a Temporary Resident please contact your nearest GP practice.

 

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
