NHS DIGITAL SHARING OPT OUT - we would like to reassure our patients that the proposed extraction of GP data by NHS Digital does NOT include any patients registered with this practice or in NHS Scotland. NHS Digital provide services in ENGLAND so are not able to access any data held in Scotland. You are not required to opt out as your data is already safe. We take patient confidentiality extremely seriously and safeguards are in place to protect this, including when data is used for research. The Community Link Worker for the practice is Sophie Carmichael. Sophie meets with patients 1:1 following a referral by practice staff, to discuss any issues that may affect their wellbeing. These issues could include:

  • Mental Health conditions

  • Carer Support

  • Mobility issues

  • Social Isolation

  • Welfare/housing advice

  • Family support

  • Employability and volunteering

  • Financial and debt management

  • Substance misuse

  • Domestic Abuse

  • Exercise

  • Food insecurity

  • Chronic health conditions 

In follow-up meetings, Sophie aims to link people with services, community groups and information which could help to improve their wellbeing.
