NHS DIGITAL SHARING OPT OUT - we would like to reassure our patients that the proposed extraction of GP data by NHS Digital does NOT include any patients registered with this practice or in NHS Scotland. NHS Digital provide services in ENGLAND so are not able to access any data held in Scotland. You are not required to opt out as your data is already safe. We take patient confidentiality extremely seriously and safeguards are in place to protect this, including when data is used for research. New Patient Registration

Leith_premises.jpgAt present our patient list is restricted and we are only registering new babies or those who are moving in with relatives already registered at the practice.

We are happy to register new patients who live within our practice boundary area. In order to undertake the registration process we require you to complete a registration form and questionnaire. Please complete an Under 5 Questionnaire for any children aged under 5 years. These forms can be downloaded below or collected from the surgery reception.

Please be thorough and complete all sections in the registration form, as unfortunately we are unable to process incomplete forms, and this may delay your registration. Your application will be processed within one to two working days.

In order to establish NHS entitlement we need to see some form of identification in person before you can be registered. Please bring along with you two items of identification, one which must include details of your current address.

The completed registration forms must be handed in between the hours of 9.00 am - 5.00 pm, Tuesday, Wednesday, Thursday and Friday.
