New Patient Registration
We are happy to register new patients who live within our practice boundary area. Please check your address on the map on the Boundary Test option above.
We do not accept new registrations for patients who are already registered with a GP practice in our area.
In order to undertake the registration process we require you to complete a registration form and questionnaire. Please complete an Under 5 Questionnaire for any children aged under 5 years. These forms can be downloaded below or collected from the surgery reception.
Please be thorough and complete all sections in the registration form, as unfortunately we are unable to process incomplete forms, and this may delay your registration. Your application will be processed within one to two working days.
In order to establish NHS entitlement we need to see some form of identification in person before you can be registered. Please bring along with you two items of identification, one which must include details of your current address.
The completed registration forms can be handed in between the hours of 8.00 am - 5.00 pm, Monday to Friday.
